they can tell you the exact answers..

if u don't want to go to ur family doc
there r always those walk-in clinics that u can go

my frd's gf did it..
my frd said it's abt $400 ... (i dun quite remember)
and they didnt need the parents/guardian's signature..
they just need the baby's mother and father's signature (so it's my frd and his gf) and that was it
they went to a hospital.. but i dunno which one

my friend said it's free if you're citizen of Canada, and if u r not, it's about $300-400.

I think there's no need for guardian's signature as long as she understands wut she's doing...
